GTAHA's Learn to Play Hockey - Initiation Program


When: Sunday September 23, 2007 9:30 am registration
First Skate is 10:30 am following registration.
The full schedule will be announced at registration
(typically it will be Saturdays and/or Sundays at 9am)

Cost: $215.00 (25 hours of ice)
Payable at time of Registration
Notes: A Parent or Guardian MUST BE
PRESENT AT REGISTRATION to sign the
Registration Form, the USA Hockey Waiver,
and the Medical Insurance Forms

Items Needed to Register:
1. Registration Fee (payment in full required)
2. Copy of Birth Certificate
3. Medical insurance card
